HOW WE CAN HELP

The services we offer

Routine Care

Routine Home Care is the most common level of hospice care. It allows patients to receive compassionate, high-quality care in the comfort of their own home — whether that’s a private residence, nursing facility, or assisted living community.

Our hospice team provides regular visits and support to manage symptoms, control pain, and ensure each patient’s physical, emotional, and spiritual comfort. The focus is on maintaining dignity, peace, and quality of life during this stage of care.

How We Help Patients and Families

  • Personalized Care Plans: We work closely with patients, families, and physicians to create a care plan tailored to individual needs and goals.
  • Skilled Nursing Visits: Our nurses monitor symptoms, manage medications, and provide expert pain and symptom management.
  • Home Health Aides: Compassionate aides assist with personal care needs such as bathing, grooming, and dressing.
  • Emotional & Spiritual Support: Social workers and chaplains help patients and families cope with emotional and spiritual challenges.
  • 24/7 On-Call Support: A nurse is available by phone or for in-person visits any time, day or night, to ensure comfort and peace of mind.
  • Education for Families: We guide families in understanding what to expect, how to provide care, and how to find comfort and support during this time.

RESPITE Care

Respite Care is a short-term level of hospice care designed to give family caregivers a much-needed break while ensuring their loved one continues to receive compassionate, professional support. Caring for someone with a serious illness can be emotionally and physically demanding — respite care provides temporary relief and peace of mind.

During a respite stay, the patient is cared for in an inpatient hospice facility, nursing home, or hospital for up to five consecutive days, while the caregiver takes time to rest, recharge, or attend to personal needs.

How We Help Patients and Families

  • Safe, Comfortable Environment: Patients receive 24-hour care in a warm, supportive setting that focuses on comfort and dignity.
  • Expert Hospice Team: Our nurses, aides, social workers, and chaplains provide the same high-quality care and emotional support as during routine home care.
  • Caregiver Relief: Families can take a few days to rest, travel, or simply rejuvenate, knowing their loved one is in trusted, compassionate hands.
  • Continuity of Care: When respite care ends, the patient smoothly returns to their home setting with no interruption in their hospice services.
Respite Care helps families stay strong and supported — because caring for a loved one also means caring for yourself.

CONTINUOUS Care

Continuous Home Care is an intensive level of hospice support provided when a patient is experiencing a medical crisis or symptoms that cannot be managed with routine home care alone. The goal is to bring relief and stability while allowing the patient to remain in the comfort of their own home or place of residence.

This short-term care is delivered by our skilled hospice team, who provide round-the-clock nursing and support until the patient’s symptoms are under control and comfort is restored.

How We Help Patients and Families

  • Around-the-Clock Nursing Support: Our nurses remain at the bedside for extended periods, providing expert monitoring and hands-on care to manage pain, shortness of breath, restlessness, or other acute symptoms.
  • Symptom Stabilization: The focus is on quickly controlling distressing symptoms to help the patient remain comfortable and avoid unnecessary hospitalizations.
  • Skilled, Compassionate Team: Licensed nurses and hospice aides work together to provide medical care, personal care, and emotional reassurance.
  • Family Guidance and Support: We educate and support family members through the crisis, offering comfort, clear communication, and peace of mind.
  • Seamless Transition Back to Routine Care: Once symptoms are managed, the patient smoothly returns to routine home care with the same trusted hospice team.
Continuous Home Care ensures that patients receive the highest level of attention and comfort when they need it most — right at home, surrounded by those they love.
